What is Care Management?
Care management consists of assessing needs, developing treatment plans and linking and managing services. These services may consist of intermittent visits or extended continuous care.
Because every situation can be unique, the elderly often need organizational help, emotional support and a clear plan of action. Moreover, our clients' adult children, who may live far away, need a local liason who can help with minor problems before they become crises. Care management is of tremendous benefit in these areas.
Getting Started
After your initial telephone contact, a Care Manager will arrange to come to your home to personally discuss your needs and concerns. Read our Pledge of Ethics.
Assessment
This assessment interview usually takes 90 minutes. Areas of evaluation include health and safety; legal and financial issues; medical history; daily living activities; and emotional and psychological functioning. You will receive a 10-14 page report with recommendations and referrals to community resources that can help.
Care Plan
After the assessment is completed, you and your care manager will develop a plan that outlines who will do what and when. Having a Care Plan to follow reduces anxiety, anticipates emergencies and provides specific step-by-step problem solving strategies.
